Foundation of precision health. Beyond standard testing identifies subclinical dysfunction. Optimal ranges differ from conventional "normal" ranges. UPDATE (August 9, 2026): Nature Aging 2026 (Yu, Chen, Feng, Li et al.) reports the most credible ultra-early MASLD stratification tool published to date: a five-protein plasma panel — FUOM, ACY1, KRT18, CDHR2 and GGT1 — identified and validated across more than 50,000 participants from Southern UK, Northern UK, EPIC-Norfolk and a Southern China inception cohort. Performance was 5-year AUC 0.838 and 16.6-year AUC 0.756, sustained longitudinally in EPIC-Norfolk (AUC 0.710) and reaching 0.912 in the Chinese cohort; integrating routine clinical data raised these to 0.904 and 0.822. Note that GGT1 is already on most comprehensive panels, so part of this signal is accessible today. Important limits: this is an observational prediction model, no trial has shown that acting on the signature changes hepatic outcomes, and the assay is not clinically standardised or reimbursed.

    Practitioner Notes

    Clinical annotations from Dr. Goel

Optimal ranges differ from conventional. Track trends over time. Annual comprehensive with quarterly targeted testing. Interpret in context of symptoms and goals. Updated August 9, 2026: For metabolic-health patients, the 2026 five-protein MASLD signature (FUOM, ACY1, KRT18, CDHR2, GGT1) is worth knowing about because the prediction horizon is 16 years — well upstream of imaging or elastography. Practically, only GGT1 is routinely available now; treat the full panel as an emerging research tool rather than something to order. Present it as risk stratification, not diagnosis, and be explicit that no intervention trial has yet been anchored to this signature.
