    Strong and growing evidence for cardiovascular support and statin-associated myopathy. A 2024 meta-analysis of 32 randomized controlled trials (3,763 heart-failure patients; BMC Cardiovascular Disorders) associated CoQ10 with 36% lower all-cause mortality (RR 0.64, 95% CI 0.48-0.85) and 50% fewer heart-failure hospitalizations (RR 0.50, 95% CI 0.37-0.67), alongside small gains in ejection fraction (+0.51%) and six-minute walk distance (+31.7 m) and no excess adverse events (RR 0.85, 95% CI 0.46-1.54). A 2024 systematic review of RCTs reported improved systolic function (ejection fraction +5.6%) with better endothelial function, exercise capacity and quality of life. A 2025 population-based cohort (Molecular Nutrition & Food Research) linked CoQ10 supplement use to lower all-cause and cardiovascular mortality. Landmark long-term data (KiSel-10) showed selenium 200 mcg + CoQ10 200 mg daily for four years reduced cardiovascular mortality (28.1% vs 38.7%; HR 0.59, 95% CI 0.42-0.81), an effect sustained at 12-year follow-up. Ubiquinol (reduced form) offers superior absorption, especially over age 40. Evidence quality is moderate and most trials are adjunctive to standard therapy.     Mechanism of Action

    Electron carrier in the mitochondrial respiratory chain (Complexes I-III), essential for ATP synthesis. Potent lipid-soluble antioxidant that protects cell membranes and LDL particles from oxidation. Endogenous synthesis and tissue levels decline with age and are further lowered by statins, which inhibit the shared mevalonate pathway.

    2024-2025: stronger cardiovascular and mortality signal for CoQ10

    A 2024 meta-analysis of 32 randomized controlled trials (3,763 heart-failure patients; BMC Cardiovascular Disorders) associated CoQ10, added to standard therapy, with 36% lower all-cause mortality (RR 0.64, 95% CI 0.48-0.85) and 50% fewer heart-failure hospitalizations (RR 0.50, 95% CI 0.37-0.67). Ejection fraction rose modestly (+0.51%) and six-minute walk distance improved by about 31.7 metres, with no increase in adverse events (RR 0.85, 95% CI 0.46-1.54).

    A separate 2024 systematic review of RCTs reported improved systolic function (ejection fraction +5.6%) alongside better endothelial function and exercise capacity, and a 2025 population-based cohort (Molecular Nutrition & Food Research) linked CoQ10 supplement use to lower all-cause and cardiovascular mortality.
