    Mechanism of Action

    Synthetic version of exendin-4, a peptide found in Gila monster venom with 53% homology to human GLP-1. Activates GLP-1 receptors to stimulate insulin secretion, suppress glucagon, slow gastric emptying, and promote satiety. Resistant to DPP-4 degradation.

    Who Is This For?

    Type 2 diabetes where cost is primary concern. Patients needing postprandial glucose control specifically. Step therapy requirements before newer agents.

    Dose

    Byetta: 5mcg twice daily for 1 month, then 10mcg twice daily, 60 min before meals. Bydureon: 2mg once weekly, any time of day regardless of meals.
