Keep Sharp argues that the brain is not on a fixed trajectory of decline, that meaningful change is possible at any age, and that the habits which protect cognition are unglamorous and available to nearly everyone. Sanjay Gupta, a neurosurgeon and medical journalist, writes with a reporter's instinct for separating what is established from what is merely promising.
The framework is five pillars: move, meaning regular physical activity, which he treats as the single best-supported intervention; discover, meaning continued learning and cognitively demanding novelty rather than brain-training games; relax, covering sleep and stress; nourish, covering dietary patterns associated with better cognitive outcomes; and connect, covering social engagement and the cognitive cost of isolation. Underlying all five is the concept of cognitive reserve — the idea that a brain with denser connections and more varied experience can sustain more pathology before symptoms appear. Later chapters address diagnosis, what to do when memory changes appear, and the practical and emotional work of caregiving.
It is written for general readers, particularly those in midlife or later who want to act on cognitive risk, and for families supporting someone with a diagnosis.
On the evidence, this is one of the more careful popular books in the category. Gupta consistently distinguishes association from causation, notes where the research is observational, and is explicit that no lifestyle program has been shown to prevent Alzheimer's disease. He is appropriately sceptical of the brain supplement market and of commercial brain-training claims, both of which are areas where the marketing far exceeds the data. The recommendations are consistent with mainstream risk-reduction guidance and with the multidomain lifestyle trials that have shown modest cognitive benefit in at-risk older adults. Nothing here is novel to a clinician, which is rather the point: it is accurate, readable and a sound entry point for someone who wants to start with what the evidence actually supports.
