Metabolical argues that most of the chronic disease burden in wealthy countries traces to what has happened to food itself, and that medicine has organised around treating the downstream symptoms while leaving the cause untouched. Robert Lustig's summary prescription is to protect the liver and feed the gut, and his target is ultra-processed food rather than any single macronutrient in isolation.
The mechanistic argument centres on fructose and on the loss of dietary fibre during processing. Lustig describes fructose metabolism in the liver, its contribution to hepatic fat accumulation and insulin resistance, and the way processing strips the fibre that would otherwise slow absorption and feed the gut microbiome. He extends this into a set of subcellular processes he identifies as common pathways to disease, including mitochondrial dysfunction, insulin resistance, inflammation and glycation.
As much as it is a nutrition book, it is an argument about industry and policy. Lustig writes at length about food manufacturers, pharmaceutical incentives, medical training and regulatory capture, and the tone is combative. Readers should expect advocacy alongside physiology.
On the evidence, the broad claim is well supported. Higher intake of ultra-processed food is consistently associated with worse metabolic and cardiovascular outcomes, and reducing added sugar is a reasonable, mainstream recommendation. The more specific claim, that fructose is the principal driver and that this mechanism explains the bulk of chronic disease, is not consensus. Controlled feeding studies suggest much of fructose's effect at typical intakes is mediated by excess calories, and the field remains divided. Lustig states his position with a certainty the evidence does not yet carry. The dietary advice that follows is nonetheless sound, whatever the mechanism turns out to be.
