    Outlive argues that modern medicine is good at treating disease once it has arrived and poor at preventing the slow diseases that actually kill most people, and that the answer is to act decades earlier, accepting more uncertainty in exchange for acting while prevention is still possible. Attia calls the reactive approach Medicine 2.0 and the proactive one Medicine 3.0.

    The framework centres on four causes of death he calls the four horsemen: atherosclerotic cardiovascular disease, cancer, neurodegenerative disease, and metabolic dysfunction including type 2 diabetes. Each develops over decades, and each has a window in which the trajectory can be changed. His tactics are exercise, nutrition, sleep and emotional health, with exercise treated as the largest single lever — aerobic base training, peak aerobic capacity, strength, and stability to protect against injury. The Centenarian Decathlon is his organising exercise: decide what you want to be physically capable of at 90, then work backwards to what that requires of you now. He argues for measuring ApoB and Lp(a) rather than LDL cholesterol alone, and for earlier and more aggressive screening than is conventional.

    The final section on emotional health is unusually candid and is the part many readers say changed most for them.

    On evidence, this is one of the strongest entries in the library. It is extensively sourced, careful to distinguish strong evidence from weak, and explicit when Attia is extrapolating — he flags his own risk tolerance rather than disguising it as consensus. Several positions go beyond current guidelines, including aggressive early cancer screening, low ApoB targets and high protein intake, and reasonable clinicians disagree with him on screening harms and cost. It is a framework for thinking, not a protocol to self-apply: much of what it recommends requires testing, prescriptions and clinical judgement.
