Plasma proteome profiling identified biomarkers for the differential diagnosis and molecular staging of neurodegenerative dementias
Bellomo, Vermunt, del Campo and colleagues report a large multicentre plasma proteomics study identifying blood biomarkers that discriminate Alzheimer's disease, dementia with Lewy bodies and frontotemporal dementia, and that support molecular staging. Most blood-based neurodegeneration work to date has been AD-versus-control; differential diagnosis across syndromes is both the harder problem and the more clinically useful one.

Key Findings
- 01
Large multicentre plasma proteomics cohort spanning AD, DLB and FTD
- 02
Identified protein signatures that discriminate between neurodegenerative syndromes, not just disease versus control
- 03
Signatures also supported molecular staging within disease
- 04
Extends blood-based neurodegeneration testing beyond amyloid/tau-centric single analytes

- Methodology
- Multicentre plasma proteomic profiling across clinically and biomarker-characterised neurodegenerative dementia cohorts with discrimination and staging analyses.
- Limitations
- Assay standardisation across platforms and prospective validation in unselected memory-clinic populations remain outstanding. Cohort-based discrimination performance typically overstates real-world performance where pre-test probability and comorbidity differ. Not yet a clinically deployable panel.
