    Acetyl-L-carnitine (ALCAR) is the acetylated form of carnitine, able to cross the blood-brain barrier more readily than plain L-carnitine. Randomized trials and meta-analyses support its use for diabetic peripheral neuropathy (e.g., the AGATHA study) and for depression in older adults, where a meta-analysis found benefits comparable to standard antidepressants in some trials. Evidence for cognitive enhancement in healthy, non-elderly adults is weaker and more mixed, and a widely cited Cochrane review of cognitive benefit in mild cognitive impairment has been criticized for relying on smaller, older, industry-associated trials.

    Mechanism of Action

    ALCAR shuttles fatty acids across the mitochondrial membrane as part of the carnitine shuttle, supporting mitochondrial beta-oxidation and ATP production, particularly in neurons and skeletal muscle. The acetyl group it carries can also be donated to acetyl-CoA and used in acetylcholine synthesis, which may underlie its effects on mood and peripheral nerve function; in diabetic neuropathy, proposed mechanisms include improved nerve regeneration and reduced oxidative damage to peripheral nerves.
