    4/10 Evidence

Ketone esters and beta-hydroxybutyrate (BHB) salts raise blood ketone levels without fasting or a ketogenic diet. Meta-analyses show reliable increases in blood BHB and a modest acute lowering of glucose, with early evidence of modest cognitive effects; athletic-performance results are mixed.

    Mechanism of Action

Ketone esters deliver BHB, which is used as an alternative fuel by brain, heart and muscle, and also acts as a signalling molecule (HDAC inhibition, NLRP3 inflammasome and GPR109A effects). Acute ketosis suppresses hepatic glucose output and lowers circulating glucose and lactate. Ketone esters raise BHB far more than BHB salts.

    Dosing Protocol

    Recommended

10g of BHB or ketone ester per serving (ketone esters typically 10-25 g; BHB salts 5-12 g)

Standard starting dose based on research

    Therapeutic Range

    5

    Minimum

    to

    25

    Maximum

g of BHB or ketone ester per serving (ketone esters typically 10-25 g; BHB salts 5-12 g)

Fasted or before cognitively/physically demanding tasks, 30 min prior; avoid late evening (may disturb sleep)

    Cycling Protocol

Use occasionally or around specific tasks rather than continuously. Not intended as a daily weight-loss strategy

Dosages are general guidelines. Individual requirements vary based on body weight, health status, and specific goals. Start with lower doses and adjust based on response.

    Benefits

    Primary Benefits

    • Reliable acute rise in blood BHB and lowering of blood glucose (2022 meta-analysis of 43 trials: BHB +1.73 mM, glucose about -0.54 mM)
    • Modest improvement in cognitive performance (2026 meta-analysis of 29 protocols, 1,117 participants; benefit linked to higher daily ketone ester doses)

    Secondary Benefits

Possible post-exercise recovery and glycogen-sparing effects (inconsistent)
    Possible appetite reduction (small trials)

    Safety Information

    Contraindications

Do not use if any of the following apply:

    • •Type 1 diabetes (ketoacidosis risk)
    • •Insulin-treated or SGLT2-inhibitor-treated type 2 diabetes without clinician oversight
    • •Pregnancy and breastfeeding
    • •Kidney disease or sodium-restricted heart failure (BHB salts)
    • •Pancreatitis or fat-metabolism disorders
    • •Eating disorder history

    Drug/Supplement Interactions

May interact with the following. Consult a healthcare provider before combining:

Insulin and sulfonylureas - risk of hypoglycaemia; monitor glucose
SGLT2 inhibitors - risk of ketoacidosis (including euglycaemic DKA); avoid combining without physician oversight
    Low-carbohydrate/ketogenic diets - additive ketosis
Sodium/potassium-restricted diets - BHB salts add mineral load
    Metformin - additive glucose lowering

    Potential Side Effects

    Some users may experience:

Nausea, diarrhoea and GI upset (common with ketone esters)
    Unpleasant taste
    Headache and dizziness
    Sodium/mineral load with BHB salts
    Hypoglycaemia in susceptible users

This is not a complete list of interactions or side effects. Individual responses vary. Always start with the lowest effective dose and monitor your response. Report any adverse effects to your healthcare provider.

    Quality Markers

Look for these specific quality indicators when sourcing this compound:

    • Specifies ketone ester (R-BHB) vs BHB salt and grams of BHB per serving
    • Prefers R-enantiomer; check racemic (D/L) content
    • Third-party tested for purity
    • Mineral content (sodium, potassium, calcium, magnesium) disclosed for salts

    Trusted Certifications

    NSF Certified
    USP Verified
    GMP Certified
    ConsumerLab Approved

Note: Supplement quality varies dramatically between brands. Cheap supplements often contain fillers, incorrect doses, or contaminants. Investing in quality products from reputable manufacturers is essential for safety and efficacy.

    Evidence Profile

    Evidence Score

    4/10

    Emerging/Anecdotal

Limited clinical data, primarily mechanistic studies or user reports

    Classification:
Emerging/Moderate for biochemical effects, Emerging for clinical outcomes. Acute BHB elevation and glucose lowering are consistent across 43 trials, but long-term effects of prolonged supplementation are poorly studied. A 2026 cognition meta-analysis (Bonnechere et al.) describes the benefit as modest, with dose dependence; seen via press summary only. Systematic reviews of athletic performance report mixed results that depend on intensity, participant type and co-ingestion strategy. Scored Emerging (4); cost and palatability limit practical use.
