Curcumin is the principal polyphenol in turmeric (Curcuma longa). On its own it is poorly absorbed, so it is usually paired with piperine (a black-pepper extract) or delivered in enhanced formulations to raise blood levels — the basis for this "with piperine" preparation. Its anti-inflammatory activity is attributed to inhibition of NF-κB and COX-2 signalling and activation of the Nrf2 antioxidant response.
The 2024–2026 evidence is strongest for joint symptoms and inflammation. An umbrella meta-analysis pooling 11 prior meta-analyses of randomized trials (Phytotherapy Research, 2024) found curcuminoid supplementation significantly reduced knee-osteoarthritis pain (VAS and WOMAC pain) and improved WOMAC function and stiffness. A 2025 Bayesian network meta-analysis of 39 RCTs in 4,599 patients (Nutrients) placed curcumin among the effective, well-tolerated nutritional options for knee OA — with no increase in adverse events versus placebo — although Boswellia ranked highest for pain. On the mechanism side, a 2025 systematic review of 21 RCTs in 1,705 knee-OA patients (BMC Complementary Medicine and Therapies) found curcumin significantly lowered serum CRP (SMD −0.91) and TNF-α (SMD −0.92), while changes in IL-6, IL-1β, ESR, and PGE-2 were not statistically significant — the pattern shown in the chart below.
For the specific curcumin-plus-piperine combination, a 2025 GRADE-assessed meta-analysis of 18 RCTs (Food Science & Nutrition) reported significant reductions in the liver enzyme AST and in IL-6, with a favourable trend for ALT that reached significance in overweight and metabolically at-risk adults. Broader cardiometabolic benefits have been reported with high-bioavailability formulations: a 2025 meta-analysis of 20 RCTs in 1,394 adults (Pharmacological Research) found nanocurcumin improved fasting glucose, HbA1c, insulin sensitivity, LDL and HDL cholesterol, diastolic blood pressure, and oxidative-stress and inflammatory markers.
Curcumin is generally well tolerated; mild digestive upset is the most common complaint, and results depend heavily on the formulation and the dose actually absorbed. Rare cases of liver injury have been reported, mostly with high-dose enhanced-absorption products, and curcumin can add to the effect of blood thinners and may reduce iron absorption.
