Become a VIPDr. Goel reviews BPC-157's human literature, the origins of the 10,000-step target, protein source and training splits.
Here's what's new at Peak Human this week. Patients are most likely to raise BPC-157, so I reviewed what the human literature actually contains, plus where the 10,000-step target came from and whether protein source matters for muscle.

FDA's December 2025 literature search counted five clinical studies, roughly 81 participants: a 2002 phase 1 rectal-enema study in 24 healthy men; a 2005 ulcerative colitis RCT available only as a conference abstract (FDA: data "inadequate to support the efficacy and safety of BPC-157"); a 2021 retrospective knee-pain review; a 2024 open-label interstitial cystitis pilot; a 2025 IV safety pilot in two people. A 2025 systematic review (Vasireddi et al., HSS Journal) found 35 preclinical and 1 clinical study and no clinical safety data. Subcutaneous, oral, nasal and transdermal routes have no human studies. This summarizes published research, not an efficacy claim; BPC-157 is not an approved drug.
My take: The animal data are interesting but are not human data. The honest answer to patients is five small studies, no subcutaneous data, and unknown risks. Counsel on that uncertainty rather than dismiss the question.
Traces to a 1965 Japanese pedometer trade name. Ding et al. 2025 (Lancet Public Health): 7,000 vs 2,000 steps/day associated with 47% lower all-cause mortality (HR 0.53, 95% CI 0.46-0.60). 2026 ACC statement (Myers et al., JACC): roughly 5,000-7,000 steps for many adults plus resistance training; observational data and consensus, not trials.
Plant isolates have lower leucine (7.1% vs 8.8%) (Gorissen 2018); with matched protein 1.6 g/kg/day for 12 weeks, 19 vegans vs 19 omnivores gained the same leg lean mass ~1.2 kg (Hevia-Larraín 2021). In adults 65-85 a 100 g beef meal gave ~47% higher MPS than a vegan meal (Pinckaers 2024).
Pelland et al. 2025 (Sports Medicine), 67 studies, 2,058 participants: more weekly volume gave more hypertrophy with diminishing returns; frequency effect on hypertrophy compatible with negligible.
Lp(a)HORIZON (pelacarsen) full data expected at AHA this month.
